SS-31 in Orlando
SS-31, also called elamipretide, is a synthetic peptide that concentrates in mitochondria, the structures inside cells that produce energy. In September 2025, the FDA granted accelerated approval to an elamipretide product marketed as Forzinity.
That approval is specific and narrow. Forzinity is approved to improve muscle strength in adults and children with Barth syndrome, a rare genetic condition, who weigh at least 30 kilograms. It is not approved for fatigue, longevity, mitochondrial optimization, cardiovascular disease, athletic performance, or general wellness. An SS-31 vial from a compounding pharmacy is not Forzinity, and accelerated approval still requires confirmatory evidence.
How It Works
Mitochondria have an inner membrane built partly from a fat called cardiolipin. The shape of that membrane affects how well cells turn fuel into energy. Elamipretide binds to cardiolipin and is thought to help maintain that structure.
In Barth syndrome, cardiolipin remodeling and mitochondrial function are disrupted, which is why researchers studied a cardiolipin-targeting drug for that disease. Extending that logic to ordinary age-related fatigue is an assumption, not a finding.
Potential Benefits
The approved indication supports one benefit in one population:
Improved muscle strength in patients with Barth syndrome weighing at least 30 kg
Outside Barth syndrome and the labeled weight range, safety and effectiveness are not established. Those uses are not supported by approval, and evidence for them is limited.
Who May Be an Appropriate Candidate
Persistent fatigue has many causes, and most can be identified. Anemia, thyroid disease, sleep apnea, depression, heart or kidney disease, medication side effects, and hormone deficiency may contribute to fatigue. Each has an established treatment.
SS-31 is not appropriate when any of the following is present:
Pregnancy or breastfeeding
Fatigue that has not been medically evaluated
Renal function must be assessed. Forzinity dosing is unchanged in adults with mild or moderate renal impairment, reduced to 20 mg once daily in adults with severe renal impairment not on dialysis, and not established for adults on dialysis or pediatric patients with renal impairment.
How It Is Administered
Route
Forzinity is approved for subcutaneous use and is not approved for intravenous use. A compounded product may be labeled differently, but no alternative route is established or FDA-approved for SS-31.
When
Forzinity is administered subcutaneously once daily at the same time each day for its labeled indication. This dosing cannot automatically be applied to compounded SS-31.
Who Does It
You do it yourself at home. We instruct you beforehand.
How Often It May Be Used
A set period of daily dosing, followed by review, rather than indefinite use. Dosing for compounded SS-31 outside the approved indication is not established, so your provider will set a course length and a review date.
Safety Considerations
Compounded SS-31 is not an FDA-approved product, and it should not be described as one based on Forzinity’s approval. The approval covers a specific product, strength, dosage form, route, and indication.
Points to understand before starting:
Accelerated approval means confirmatory trials remain outstanding
Injection site reactions are the most commonly reported effect
Long-term safety outside the studied population is not established
Compounded peptides carry purity, characterization, and immune response concerns
Report injection site reactions that worsen, and tell your provider about every medication and supplement you take.
